How we handle what you share.

When you submit the contact form, we collect the details you provide (name, work email, company, role, stage, phone, areas of interest, and your message). We use that information to follow up on your inquiry and to understand how CHI can help.

We do not sell this information. We do not add you to marketing lists from this form. Inquiries may be stored securely and emailed to our team at hello@communityhealthinternational.com.

If you want information removed, email us at the same address and we will take care of it.

Patient information

Do not send patient names, record numbers, or clinical details through this website. If we later support work that involves protected health information, we operate only in the systems you authorize, under your access controls, and we will execute a Business Associate Agreement when the engagement requires it.
